Payments are due at the time of services. Our office accepts checks, cash, as well as MasterCard, Visa and Discover credit cards.
Please bring your insurance cards at the time of service so that they may be scanned into our system. Office visit insurance co-payments are due at the time of service.
Self pay patients are asked to pay for the office visit at the time of their service. In addition, payment for any diagnostic tests will also be due at this time. Our billing department would be happy to discuss financial arrangements prior to any visit. Please call 314-633-8575 option 7, or ask for the Billing Department.
Refractions (glasses prescription exam) are considered routine eye care and are frequently not covered by insurance. Therefore, the fee for this service is the patient’s responsibility.
Contact lens related services are not covered by medical insurance. They are billed separately and may include a contact lens fitting fee, examination fee, and cost of replacement lenses.
Most HMO insurance plans require a referral to be seen by a specialist. We are considered specialty care, so a referral is required for every visit. It is the patient’s responsibility to contact their primary care provider prior to the visit to request a referral be sent to our office. This can be faxed directly to us at (314)743-8399.
